Malaysia is South-East Asia's value destination, with regulated hospital fees and almost two million healthcare travellers a year. Singapore ranks among the world's best — at a price to match. India sits between them on quality rankings and well below both on cost for most major surgery.
Malaysia: regulated and regional
Malaysia's healthcare travel grew from 643,000 visitors and RM530 million in 2011 to 1.84 million travellers and RM3.34 billion in 2025. Most patients are regional — Indonesians above all — coming for check-ups, cardiology, oncology, fertility and orthopaedics. Private hospital fees are set within a government fee schedule, which makes prices predictable.
Singapore: top quality, top price
Singapore ranks second of 46 destinations in the Medical Tourism Index, with excellent hospitals, research and safety. It is among the most expensive places in Asia for treatment, and it attracts patients for whom cost matters less than speed and reputation.
Where India stands out
- Cost for major surgery: heart, transplant, cancer and joint surgery are usually far cheaper in India.
- Depth of specialists: very large teaching hospitals and high case volumes in every speciality.
- For patients from Africa, the Gulf, Central and South Asia: India is often closer, with familiar food, languages and communities.
- Recovery: yoga, Ayurveda and the Himalayan foothills near Chandigarh.
Side by side
- Quality ranking (MTI 2020–21): Singapore 2nd · India 10th · Malaysia outside the top 15.
- Patients: Malaysia 1.84 million (2025) · India 644,387 (2024) · Singapore does not publish a comparable recent count.
- Typical strength: Malaysia — value and regional access · Singapore — complex care at premium prices · India — complex care at low prices.
- Language: English widely used in all three.

Everyday differences for families
- Food: Malaysia is Muslim-majority and halal food is the norm; in India halal, vegetarian and Jain food are easy to find, and hospitals cater for all three.
- Weather: Malaysia and Singapore are hot and humid all year; north India has real seasons — cool winters, hot early summers and a monsoon.
- Cost of stay: hotels and food for the family usually cost less in India than in Singapore.
- Language: English works in all three; Indian hospitals often also have Arabic, French, Russian and Swahili-speaking staff or interpreters.
- Faith: India's mosques, gurdwaras, temples and churches mean most families find a familiar place to pray.
What patients travel for
- Malaysia: health screening, cardiology, oncology, fertility and orthopaedics for regional patients, with fees within a government schedule.
- Singapore: complex cancer care, cardiac and neurosurgery, transplants and paediatric specialities, mostly for patients whose cost is covered by insurers or employers.
- India: heart bypass and valve surgery, kidney and liver transplants, joint replacement, spine and neurosurgery, cancer treatment including radiotherapy, IVF, eye surgery and dental care — at prices most families can plan for.
Each country is strong in its own way; the right one depends on the condition, the budget and where the family lives.
If you are an Indonesian family needing a check-up, Malaysia may be the easiest answer. If you are a Kenyan family facing a heart operation, India is likely the better fit.
How to decide
- Live in Indonesia or nearby?→ Malaysia is closest and good value
- Need the very top and cost is secondary?→ Singapore
- Need complex surgery on a budget?→ India
- From Africa, Gulf or Central Asia?→ India is usually closer and cheaper
- Still unsure?→ two specialist opinions, then compare written estimates
Whichever you choose, compare written, itemised estimates in the same currency, and add flights, accommodation and a companion. Ask how follow-up will work once you are home, and make sure you leave with a full discharge summary in English and a list of every medicine with its generic name.
We are honest about this: if Malaysia or Singapore suits you better, we will say so. If India is right, we match you to the best doctor for your case and handle the journey end to end. Start with a US$99 consultation — two calls — or read the full destination comparison.
