Bariatric Surgery in India: Sleeve vs Bypass, Eligibility

Weight-loss surgery in India: sleeve vs bypass with 10-year results, BMI eligibility including Asian thresholds, diet phases, costs and follow-up.

He had tried every diet, twice. The weight always came back, and with it the blood-sugar readings, the knee pain and the breathless stairs. Weight-loss surgery is not a shortcut or a moral failure — it is a medical treatment for a medical disease. Here is how the two main operations compare, who qualifies, and what life looks like after.

Who is eligible?

International guidelines updated in 2022 by the American Society for Metabolic and Bariatric Surgery and the International Federation for the Surgery of Obesity recommend surgery for adults with a BMI of 35 or more, whether or not they have related illnesses, and suggest considering it from a BMI of 30 for people with metabolic disease such as type 2 diabetes.

Because people of South Asian and East Asian origin develop diabetes and heart disease at lower weights, the same guidelines say thresholds should be lower: surgery should be offered from a BMI of 27.5. That matters for many of our patients from South Asia, the Gulf and the diaspora.

BMI is a rough tool — it does not see where fat sits or how much muscle you carry — but it is the yardstick guidelines use. Your surgeon will also look at waist size, blood sugar, blood pressure, sleep apnoea and joint pain, because surgery is as much about these illnesses as about the number on the scale.

BMI is only the start. The team also checks your heart, lungs, hormones and mental well-being, and whether you are ready for the lifelong changes in eating that surgery needs.

Newer weight-loss injections have changed the conversation, and they help many people. A good team will discuss them honestly alongside surgery — their cost, side effects and what happens if you stop — so you can choose with the full picture. We do not push surgery.

Sleeve or bypass?

  • Sleeve gastrectomy: about three-quarters of the stomach is removed, leaving a narrow tube. Simpler and quicker; no rerouting of the intestine. It can worsen acid reflux.
  • Roux-en-Y gastric bypass: a small stomach pouch is joined directly to the small intestine. Slightly more weight loss on average and often better for reflux, but needs lifelong vitamin supplements.
  • One-anastomosis (mini) bypass: a simpler variant offered by many Indian surgeons; ask about its pros and cons for you.

The best long-term evidence comes from SLEEVEPASS, a Finnish randomised trial that followed patients for ten years.

Mean excess weight lost; oesophagitis (reflux inflammation) seen on endoscopy; type 2 diabetes remission. Salminen P. et al., JAMA Surgery, 2022. The diabetes difference was not statistically significant.

Both operations worked; bypass gave somewhat more weight loss and far less reflux inflammation, while sleeve was simpler. Your surgeon will weigh your reflux, diabetes, medicines and preferences.

The diet phases after surgery

  1. Week 1 — clear liquids: water, clear soups, sugar-free drinks in small sips.
  2. Weeks 2–3 — full liquids: protein shakes, thin dal, buttermilk, smooth soups.
  3. Weeks 3–4 — soft, puréed foods: mashed dal, soft paneer, eggs, blended vegetables.
  4. From about week 5 — soft solids, then normal textures: small plates, protein first, chewing slowly, no drinking with meals.

Portions after surgery are tiny at first — often a few tablespoons — and hunger is usually much weaker for months. The skill to learn is eating slowly, stopping at the first sign of fullness, and getting enough protein every day — your dietitian will set your personal target. Sugary drinks and fried snacks can cause “dumping”: sweating, cramps and a racing heart soon after eating, especially after bypass.

Your dietitian will give a plan in your own cuisine. Indian food adapts well: dals, curd, paneer, eggs, chicken and lentil soups are protein-rich staples.

Your trip and follow-up

We plan about two weeks in India: tests and consultations, surgery with two or three nights in hospital, then a week of recovery with dietitian visits before you fly. Chandigarh, an hour from Delhi by air, is a calm, green city for those first walks.

  • Daily walking from the first day, building up steadily.
  • Vitamin and mineral supplements as prescribed — for life after bypass.
  • Blood tests at 3, 6 and 12 months, then yearly, with your doctor at home.
  • Online check-ins with your surgeon and dietitian; a support group helps many people.
  • Women are usually advised to avoid pregnancy while weight is falling quickly, often for the first year or more; ask your surgeon.

Prices are per person and exclude flights. Read our weight-loss surgery guide, see costs side by side, or explore wellness retreats for a gentle restart. Then book a consultation — US$99 includes two calls.

What BMI do I need for bariatric surgery?

Guidelines recommend surgery from BMI 35, or from 30 with metabolic disease. For people of Asian origin, surgery should be offered from BMI 27.5.

Is a gastric sleeve or bypass better?

In a 10-year trial, bypass led to slightly more weight loss and much less reflux inflammation; sleeve is simpler. Your surgeon will advise based on your health.

How long do I need to stay in India for bariatric surgery?

About two weeks: tests, surgery with two or three nights in hospital, and a week of recovery with dietitian visits.
