How to Compare Two Hospital Quotes, Line by Line

Two hospital quotes, two different totals? A worked example and a ten-line method to compare treatment estimates in India fairly, before you pay anything.

The two emails arrive on the same evening. Hospital A: US$6,200. Hospital B: US$5,400. The family's first instinct is to reply to Hospital B. Then someone asks, "Is that for the same thing?" — and the evening becomes a spreadsheet. This guide is that spreadsheet, done calmly, with a worked example you can copy.

Why the cheaper quote is often not cheaper

Hospitals price the same operation in different shapes. One quotes a package for a set number of days; another lists each item. One assumes a private room, the other a twin-sharing room. One includes the implant, the other prices it separately. Our earlier piece on why estimates become confusing explains the causes. This one is about the method: putting two quotes on the same footing so the totals finally mean something.

There is also a human reason to slow down. When a family member is unwell, the lower number feels like relief, and relief is hard to question. A few careful emails now protect both the budget and the trust you will need in the hospital later.

The rule is simple. Never compare the bottom lines first. Compare the lines, fill the gaps with a written question, then compare the totals.

A worked example, line by line

Below is an illustrative example for a knee replacement. The numbers are invented for teaching and are not any hospital's prices. What matters is the method.

Teaching example only — not real hospital prices. Package prices are per person and exclude flights.

Notice what happened. Hospital B was not dishonest: its quote was simply built differently. Once the days, room, implant and tests matched, the "cheaper" option cost more. Sometimes the opposite happens, and the package hides items you will never use. Either way, you only find out by going line by line.

The ten lines to line up

How we compare two quotes
  1. Same procedure?Check the operation's exact name, one side or both, and the approach.
  2. Same days?Ward days and ICU days included, and the price of one extra day.
  3. Same room?Room category can change other charges too.
  4. Same implant or device?Brand, model and whether it is priced within government caps.
  5. Same tests and doctors' fees?Pre-operative tests, anaesthetist, surgeon's assistants.
  6. Same aftercare?Physiotherapy, dressing changes, medicines to take home, follow-up visits.
  7. Then compare totalsOnly once every gap has a written answer.
  1. Procedure: exact name, which side, and the technique.
  2. Hospital days: ward and intensive care, and the price of each extra day.
  3. Room category: and whether it changes doctors' visit fees.
  4. Surgeon and anaesthetist fees: included, or billed separately?
  5. Implant or device: brand and model. For coronary stents and knee implants India sets ceiling prices, so the device line should be easy to check.
  6. Pre-operative tests: blood tests, scans, cardiology clearance.
  7. Medicines and consumables: during the stay and to take home.
  8. Physiotherapy: in hospital and after discharge.
  9. Follow-up: review visits, stitch removal, video calls after you fly home.
  10. Money terms: currency, deposit, refund rules and what happens if plans change.

The one email that saves the most money

When a line is missing, do not guess. Send each hospital the same short email: "Please confirm in writing whether your estimate includes the following, and the price if not." Then paste your list. Hospitals that answer clearly and quickly are telling you something about how they will treat you later, too.

India's draft Charter of Patients' Rights lists transparency in rates and an itemised bill among a patient's rights, so asking is normal, not rude.

Two more habits help. First, convert both quotes into one currency on the same day, and note whether the hospital will bill you in rupees or dollars — exchange rates can move a few per cent between quote and payment. Second, ask what happens if you need one more night or a higher level of care. A hospital that names a clear price for an extra ward day, and for a day in intensive care, makes your worst case knowable. That number is often more useful than the headline total.

Finally, remember what no quote includes: your flights, your companion's meals, and the weeks of recovery before you fly home. Put those in your own column so the family sees the whole trip, not just the hospital bill. Read more in our guide to patient rights in India, and see whether a package or itemised price suits your case.

We do this comparison for every family we help, and share the side-by-side before anyone pays. Compare typical costs on our savings page, or send us your two quotes.

प्रश्नहरू

How do I compare hospital quotes for surgery abroad?

Compare line by line before comparing totals: procedure, days, room, fees, implant, tests, medicines, physiotherapy, follow-up and payment terms. Ask each hospital in writing to fill any gap.

Why is one hospital quote so much cheaper than another?

Usually because it includes less: fewer days, a shared room, a cheaper implant or no tests. Once both quotes cover the same items, the difference often shrinks or reverses.

Are implant prices regulated in India?

Yes for some devices. India's NPPA sets ceiling prices for coronary stents (revised from April 2026) and knee replacement implants (extended to 15 November 2026 or further orders).
