Shoulder Dislocation and Rotator Cuff Surgery in India

Shoulder dislocation and rotator cuff tears in athletes: who needs surgery, Bankart vs Latarjet, recovery milestones and published costs in India.

The first time, the shoulder went back in on the touchline. The second time, it happened reaching for a rebound in training. Now the athlete sleeps with the arm tucked in and thinks twice before every tackle, every serve, every throw. Shoulder injuries steal confidence before they steal seasons. Here is how they are treated, and what to plan if you come to India.

Two different shoulder problems

Athletes often lump all shoulder pain together, but the two big injuries behave very differently.

  • Dislocation and instability: the ball of the shoulder slips out of its shallow socket, usually forwards. It often tears the labrum — the rim of cartilage that deepens the socket (a Bankart lesion) — and can chip bone from the socket or the ball. It is mainly a young athlete's injury, common in rugby, football goalkeepers, wrestling, judo, basketball and volleyball.
  • Rotator cuff tears: the four tendons that hold the ball centred and lift the arm fray or tear. In throwers, swimmers and fast bowlers this is often gradual overuse; in older players it can follow a fall. Partial tears are common and many settle with rehabilitation.

An MRI — sometimes an MR arthrogram, with dye in the joint — shows which problem you have. Send the images, not only the report. A CT scan may be added if the surgeon suspects bone loss from the socket, because that changes which operation is right.

After a dislocation: the age question

Age is the strongest predictor of the shoulder coming out again. In Hovelius's long Swedish follow-up, 72% of people whose first dislocation happened before age 22 had a recurrence over 25 years, compared with 27% of those over 30.

Recurrence after a first shoulder dislocation, 25-year follow-up (%)
First dislocation under 2272%First dislocation over 3027%

Hovelius et al., as summarised in Rev Bras Ortop 2017;52(5):513–520. A 2015 BJSM meta-analysis found 39% recurrence overall, with odds 13 times higher at age 40 and under.

That is why surgeons often discuss stabilisation early for young athletes in contact or overhead sport, while a first dislocation in an older, less active person is usually treated with physiotherapy. Each recurrence can add damage to the bone and labrum, so the decision deserves a proper review rather than a wait-and-see that drifts on for years.

The common operations are:

  • Arthroscopic Bankart repair: keyhole surgery that re-attaches the labrum to the socket with small anchors.
  • Latarjet procedure: an open operation that moves a small piece of bone to rebuild the front of the socket, used when bone loss is significant or a repair has failed.
  • Remplissage: an add-on that fills a dent in the ball (a Hill-Sachs lesion) with nearby tendon.

Rotator cuff tears in athletes

For a young overhead athlete, rotator cuff pain is often a partial tear or tendon irritation driven by load, posture and shoulder-blade control. The first treatment is usually a focused programme: load management, scapular strength, throwing or stroke mechanics, and gradual return.

Surgery is discussed for full-thickness tears, tears after a clear injury, and pain that does not improve after a sensible rehabilitation period. Arthroscopic repair re-attaches the tendon to bone with anchors. Your surgeon will talk about the size of the tear, the quality of the tendon and your sport; ask what return to the same level looks like for players like you, and what the honest uncertainty is.

Your stay and the road back

Most shoulder stabilisations and cuff repairs are planned as short admissions, but the protected-healing phase is long. Plan to stay one to two weeks in India for surgery, wound checks and the first physiotherapy sessions, or longer if you want a supervised early-rehab block.

A typical shoulder surgery trip
  1. ReviewMRI or MR arthrogram, CT if bone loss is suspected
  2. Surgerykeyhole Bankart or cuff repair; Latarjet if needed
  3. Sling phaseprotected movement as your surgeon directs
  4. Early rehabrange of motion, then shoulder-blade control
  5. Strength and sportprogressive loading, then sport-specific drills
  6. Return-to-play testsstrength and confidence before contact or throwing

Timelines vary with the operation, the tear and the sport — your surgeon sets yours.

Return to contact or overhead sport usually takes months, not weeks. Ask your surgeon to write down the milestones, and share them with your club physio. If you are a tested athlete, check every painkiller and injection first — read anti-doping and treatment abroad.

Costs and what to ask

CARE Hospitals publishes ₹90,000 to ₹1,80,000 for rotator cuff surgery in India — about US$950 to US$1,900 at ₹95 per US$ — and says the figure is a guide, not a quote. Shoulder stabilisation sits in a similar band, but the number of anchors and any bone-block procedure change the price. Package prices are per person and exclude flights.

  • Which operation do you recommend, and why that one for my sport?
  • How many anchors or implants, and which brand?
  • What is the re-dislocation or re-tear risk for someone like me?
  • When can I run, lift, throw and play contact?

Start with our sports medicine guide, read the athlete's planning guide, or book a US$99 consultation — two calls with a specialist who will look at your images.

प्रश्नहरू

Do I need surgery after my first shoulder dislocation?

Not always. Young athletes in contact or overhead sport have a high chance of recurrence, so surgeons often discuss stabilisation; older or less active people usually start with physiotherapy.

What is the difference between Bankart and Latarjet surgery?

A Bankart repair re-attaches the torn labrum by keyhole surgery. A Latarjet moves a small piece of bone to rebuild the socket, used when bone loss is significant or a repair has failed.

How much does rotator cuff surgery cost in India?

CARE Hospitals publishes ₹90,000 to ₹1,80,000 (about US$950–1,900) as a guide. Your written estimate depends on the implants, the room and the length of stay.
