Achilles Tendon Rupture: Surgery or Boot? Treatment in India

Achilles tendon rupture explained for athletes: surgery vs a functional boot, what a 526-patient trial found, recovery and planning treatment in India.

Many people describe the same moment: a sharp kick to the back of the heel, a turn to see who did it — and nobody there. An Achilles tendon rupture often happens on a push-off, a lunge or a jump, in a sprinter, a squash player, a footballer or a 40-year-old back on the court after years away. The good news is that there is more than one sound way to treat it.

How a rupture shows itself

The Achilles is the thick tendon joining the calf muscles to the heel bone. When it ruptures, most people feel a pop or a blow, then weakness pushing off. Some can still walk with a limp, which is why ruptures are sometimes missed at first.

A doctor can usually diagnose it by examination. In the Thompson test, you lie face down and the doctor squeezes your calf: if the tendon is intact, the foot points down; if it is ruptured, it barely moves. An ultrasound or MRI can confirm the gap and help plan treatment.

Speed matters. Whichever treatment you choose, it should start within days, so the tendon ends can heal close together. If you are injured abroad, get the leg into a boot or cast with the toes pointed down, and ask about blood-clot prevention before any long flight.

Three treatments, one honest comparison

  • Non-surgical (functional) treatment: a boot with heel wedges keeps the foot pointed down, with early, protected weight-bearing and a structured rehabilitation plan.
  • Open repair: the surgeon stitches the tendon ends together through an incision at the back of the ankle.
  • Minimally invasive repair: the tendon is stitched through small cuts, aiming for a smaller wound.

In 2022 the New England Journal of Medicine published a trial of 526 adults in Norway who were randomly assigned to one of the three. At 12 months, the patient-reported Achilles score was similar in all groups. The real differences were in the side effects.

Myhrvold et al., NEJM, 14 April 2022 (526 adults), as reported by HealthDay/Physician's Weekly.

Surgery lowered the chance of the tendon tearing again; non-surgical treatment avoided wound and nerve problems. An accompanying editorial concluded that none of the standard treatments is clearly better for unselected patients.

What athletes usually choose

Trials like this include many recreational players. For an elite sprinter, jumper or footballer, a re-rupture can cost a season — so many athletes and their surgeons lean towards repair to cut that risk, accepting the small chance of a wound or nerve problem. Others, especially those with diabetes, smokers, or people wary of surgery, choose a well-run functional programme.

Either way, rehabilitation decides much of the result. The calf can stay weaker than the other side for a long time, and that weakness is what slows your push-off and your sprint. Ask your surgeon and physio how they will measure calf strength, and what they need to see before you run, jump and return to full sport.

What about injections to speed healing? A large UK trial (PATH-2) found that platelet-rich plasma did not improve function compared with placebo for people treated without surgery. Read PRP and injections: the evidence.

Planning treatment in India

Achilles rupture: a typical path
  1. Confirm the ruptureexamination, ultrasound or MRI
  2. Choose with your surgeonfunctional boot, open or minimally invasive repair
  3. First two weekswound checks and boot with heel wedges
  4. Protected loadingweight-bearing in the boot as advised
  5. Out of the bootcalf strength, balance and walking pattern
  6. Back to sportrunning, jumping and sport drills after strength tests

Your surgeon sets the timeline; return to full sport commonly takes many months.

If you choose surgery in India, plan around ten days to two weeks for the operation and the first wound check before flying. Your surgeon decides when it is safe to fly and what blood-clot prevention you need. You will leave with an English report, the boot settings, a week-by-week rehab plan and contact details for questions from your physio at home.

If you choose non-surgical treatment, a specialist review of your scan can still help — to confirm the gap is small enough, and to give you a clear plan to follow at home.

What to ask before you decide

  • How big is the gap, and does it close when my foot points down?
  • What re-rupture and wound risks do you see in your own patients?
  • Which boot and heel wedges, and when do they change?
  • How will you measure my calf strength, and what targets must I reach?
  • Which painkillers and injections are allowed if I am a tested athlete?

The hospital will give you a written estimate. Package prices are per person and exclude flights. Start with our sports medicine guide, read the athlete's planning guide, or book a US$99 consultation — two calls with a specialist who will look at your scans.

Does a ruptured Achilles tendon always need surgery?

No. A large 2022 trial found similar 12-month function with a functional boot, open repair or minimally invasive repair. Surgery lowered re-rupture; non-surgical care avoided nerve and wound problems.

Why do many athletes choose Achilles surgery?

Because repair cut the re-rupture rate from 6.2% to 0.6% in the trial, and a re-rupture can cost an athlete a season.

Can I fly after an Achilles rupture?

Only when your doctor says so. Lower-leg immobilisation raises blood-clot risk, so ask about prevention before any long flight.
