The haematologist says the word “transplant”, and then a second question follows straight away: who could be the donor? A bone marrow transplant — more precisely a stem cell transplant — can cure some blood cancers and blood disorders. It is also one of the most demanding treatments in medicine, for the patient and the family. This guide explains the types, the donor search, the time it takes and the cost.
Three kinds of transplant
- Autologous. Your own stem cells are collected, stored, and returned after high-dose chemotherapy. Used mainly for multiple myeloma and some lymphomas.
- Allogeneic. Stem cells come from a matched donor — often a brother or sister, or an unrelated volunteer. Used for many leukaemias, aplastic anaemia and inherited conditions such as thalassaemia major.
- Haploidentical (half-matched). A parent, child or sibling who is a half match donates. This has opened transplant to many families who have no full match.
Whether a transplant is right at all — and when — depends on the disease, how it has responded to treatment, and the patient’s overall health. Some conditions are best treated without a transplant. Our US$99 consultation includes two calls, so a transplant haematologist can review the reports and a second specialist can confirm the advice.
Finding a donor
Matching uses HLA typing, a blood or cheek-swab test. Each full brother or sister has about a one-in-four chance of being a full match, so siblings are tested first. If there is no match, the team may search unrelated donor registries or consider a half-matched family member. Donors are usually given injections for a few days and then give stem cells from the blood through a machine, much like a long blood donation; marrow is sometimes taken from the hip under anaesthetic instead.
A family donor travels with the patient. Everyone needs the right visa: the patient an e-Medical visa, the donor and carers e-Medical Attendant visas. Our medical visa guide explains how.
What it costs
| USA · allogeneic, billed charges | 1,261,800 |
|---|---|
| USA · autologous, billed charges | 577,000 |
| India · half-matched | 21,053 |
| India · allogeneic | 18,947 |
| India · autologous | 15,789 |
US: Milliman 2025 estimated average billed charges, 30 days before to 180 days after transplant, including medicines — not like for like. India: Yashoda Hospitals approximate costs (May 2026), converted at ₹95 per US$. A 2014 study at BLK Hospital, Delhi, found median costs of about US$12,500 (autologous) and US$18,000 (allogeneic). Indian figures are hospital-published estimates, not quotes; your own estimate comes after a doctor reviews your reports.
The bill grows with complications, such as infections, graft-versus-host disease or longer intensive care, so ask what the package covers and what a typical extra week costs. Long stays mean accommodation is a real part of the budget: our Hoppers long stays start from US$999 a month with three meals, and long-term care from US$1,299 a month per patient. Package prices are per person and exclude flights.
The months of a transplant
- Reports and HLA typingPatient and siblings tested; results shared
- Transplant team reviewIs transplant right, which type, which donor?
- Arrival and work-upHeart, lung, kidney, dental and infection checks for patient and donor
- ConditioningChemotherapy, sometimes with radiotherapy, over several days
- Transplant dayStem cells given through a drip, like a transfusion
- EngraftmentAbout 2–4 weeks in a protected unit until blood counts recover
- Close follow-upFrequent clinic visits while living nearby
- Going homeUsually 2–3 months after transplant, with a detailed plan

Families from South Asia and the Middle East often ask about thalassaemia major, where regular blood transfusions are needed from childhood. A matched-sibling transplant in a young child can offer a cure, and Indian centres have long experience with it. Ask early: results are generally better when the transplant is done before iron overload damages the organs.
Plan the family’s roles before you leave home. One adult carer should be free to stay throughout, and a second can relieve them part-way. The carer learns about medicines, hygiene, food safety and warning signs, and becomes the team’s partner during the long weeks after discharge.
Autologous transplants are shorter; many patients go home about four to six weeks after arrival. Chandigarh is about an hour from Delhi by air. During recovery, crowds and infection risks matter more than sightseeing, so a quiet, clean apartment close to the hospital is worth more than a view.
Questions to ask the transplant team
- Is a transplant the best option now, or later, or not at all?
- Which type, and who is the best donor for me?
- How many transplants of this kind does your unit do each year?
- What does the package cover, and what costs extra?
- How long must we stay, and what follow-up can happen at home?

Yashoda Hospitals publishes approximate costs of ₹15 lakh for autologous, ₹18 lakh for allogeneic and ₹20 lakh for half-matched transplants (about US$15,800–21,100). Complications add to the bill.
Who can be a bone marrow donor for my family member?
Brothers and sisters are tested first; each has about a one-in-four chance of a full match. A half-matched parent, child or sibling, or an unrelated registry donor, may also be possible.
How long do we need to stay in India for a bone marrow transplant?
Usually four to six weeks for an autologous transplant and about two to three months after an allogeneic transplant.
