Liver Transplant in India: Living Donors, Law and Costs

Liver transplant in India for foreign patients: who needs it, living donors, the THOTA rules and authorisation committee, costs and a 2–3 month plan.

A son sits beside his father’s hospital bed, reading about a word that changes everything: living donor. In a living donor liver transplant, part of a healthy relative’s liver replaces the failing one, and both halves grow back. India now does more of these operations than any other country. Here is how it works for families from abroad — the medicine, the law and the money.

Who needs a new liver

A transplant is considered when the liver is failing and other treatment cannot rescue it: advanced cirrhosis with fluid build-up, bleeding or confusion; some liver cancers that have not spread; sudden (acute) liver failure; and, in children, conditions such as biliary atresia. Doctors use blood-test scores such as MELD (PELD for children) to judge how urgent it is.

Before transplant comes everything that might make it unnecessary or safer: stopping alcohol completely, antiviral medicines for hepatitis B or C, treating complications, and nutrition. Some people are too unwell, or have other conditions, that make transplant unsafe; an honest team will say so. Our US$99 consultation includes two calls, so a hepatologist can review your reports and a transplant surgeon can give a second view.

Living donors: the heart of the decision

Most transplants in India use a living donor. The donor gives part of their liver — the right or left lobe for an adult, a smaller segment for a child — and the remaining liver regrows over the following weeks and months. The recipient’s new piece grows too.

Donors are carefully assessed: blood group, general health, liver scans to check size and fat, and a psychological review. Donating is major surgery with real, if uncommon, risks. A good team protects the donor first and lets them step back at any point, confidentially, without explanation to the family.

Most donors are back on their feet within days and home from hospital in about a week, though full strength takes longer. Donors should plan time off work after returning home and keep their own follow-up appointments; their health matters as much as the recipient’s.

The law for foreign patients

Transplants in India are governed by the Transplantation of Human Organs and Tissues Act and its 2014 Rules. For foreign nationals, three rules matter most:

  • Near relatives only. A living Indian donor may give to a foreign recipient only if they are a near relative, so most families bring a related donor from home.
  • Embassy certification. A senior official of your embassy in India certifies the donor–recipient relationship on Form 21.
  • Committee approval. The hospital’s authorisation committee must approve the transplant before surgery, after interviewing the donor and family.

Deceased-donor livers go to foreign patients only when no Indian patient is available. We never arrange unrelated or paid donors. Our full explainer, organ transplants in India for foreign patients, sets out the documents step by step.

PACE Hospitals: India ₹18.5–26.75 lakh. CARE Hospitals: ₹14.5–22.5 lakh. Converted at ₹95 per US$. For comparison, Milliman estimates average US billed charges of US$1,017,800 per liver transplant (2025), including the months before and after and medicines — not like for like. Indian figures are hospital-published estimates, not quotes; your own estimate comes after a doctor reviews your reports.

Ask what the package includes: donor and recipient evaluation, both operations, intensive care, hospital days, and medicines for how long. Anti-rejection medicines continue for life and should be part of your budget at home. Package prices exclude flights.

Most families arrive through Delhi; Chandigarh is about an hour by air. Two people recovering from major surgery need a calm, clean place to stay near the hospital — our Hoppers long stays start from US$999 a month with three meals.
