Deep Brain Stimulation for Parkinson’s in India

Deep brain stimulation for Parkinson’s in India: who it helps, what it cannot do, published costs by system, programming and a 3–4 week plan.

For years the tablets worked. Then the good spells between doses grew shorter, and the shaking or the stiffness came back sooner each day — or the medicine itself brought restless, writhing movements. For some people with Parkinson’s disease, deep brain stimulation (DBS) can smooth out those swings. It is not a cure, and it is not for everyone. Here is an honest guide.

What DBS is

In DBS, a neurosurgeon places thin wires (leads) in a precise target deep in the brain. They connect under the skin to a small battery-powered stimulator in the chest, much like a pacemaker. Gentle electrical pulses calm the faulty signals behind tremor, stiffness and slowness. The settings can be adjusted from outside the body, and the system can be switched off.

Many centres keep the patient awake for part of the lead placement, because the brain itself feels no pain and the team can test the effect on tremor and stiffness in real time. Others work under general anaesthesia using detailed imaging. Ask which approach your team uses and why; both are established.

It is also used for essential tremor and dystonia. This story focuses on Parkinson’s, where most questions come from.

Who is a good candidate

DBS tends to help most when a person still responds well to levodopa but struggles with “off” periods, medicine-induced extra movements (dyskinesia), or tremor that medicines do not control. A movement-disorder neurologist decides, usually after a formal test of how well you respond to your medicine, a brain MRI, and a check of memory and mood. Significant memory problems or untreated depression can make DBS unsuitable.

Before surgery, ask whether your medicines have been fully adjusted. Changes to timing and dose, other Parkinson’s medicines, physiotherapy and exercise help many people, and some find they do not need DBS yet. Our US$99 consultation includes two calls, so a neurologist and a neurosurgeon can each review your case.

Global: Bishay et al., mean total cost of DBS surgery across 26 studies, inflation-adjusted to April 2022 US$. India: PACE Hospitals estimates by system type, Hyderabad (June 2026), upper end of each range, converted at ₹95 per US$. Indian figures are hospital-published estimates, not quotes; your own estimate comes after a doctor reviews your reports.

The device is the largest part of the bill. Rechargeable and directional systems cost more upfront; non-rechargeable batteries need replacing every few years, which is a further operation and cost. Ask which brand is offered, and — crucially — whether a neurologist in your home country can program that brand.

How a DBS trip runs

Deep brain stimulation, step by step
  1. Records and videosMedicine list, neurologist letters, short phone videos of your ‘on’ and ‘off’ states
  2. Assessment in IndiaLevodopa challenge test, MRI, memory and mood review
  3. Team decisionNeurologist and neurosurgeon agree DBS is right — or advise against it
  4. SurgeryLeads placed, often in two stages a few days apart; battery in the chest
  5. ВосстановлениеA few days in hospital, then rest nearby
  6. First programmingSettings switched on and adjusted; medicines often reduced
  7. Hand-over planDevice details, settings and a contact for your home neurologist
  8. Fly homeWith your device card and the surgeon’s clearance

We usually plan three to four weeks in India. Chandigarh is about an hour from Delhi by air, and its parks and the promenade at Sukhna Lake offer flat, quiet walking — useful when the team wants to watch how you move as settings change.

Life with a stimulator

Many people need fewer tablets after DBS, and their days become more predictable. Balance, speech and memory problems often do not improve and can sometimes worsen, so set expectations with your team. You will carry a device card, take care with some medical scans and airport security, and see your neurologist for setting changes over time.

Family members play a large part in DBS. They often notice changes in mood, speech or behaviour before the patient does, and they help keep the medicine diary that guides programming. Bring the person who spends most time with the patient to the assessment if you can — the team will want to hear from them, and they will want to understand the device.

Parkinson’s is a long journey, and families need support too. Our long-term care programme and the story on care for dementia and Parkinson’s describe what we can arrange.

Вопросы, которые стоит задать

  • Do my symptoms respond to levodopa, and how well?
  • Which target in the brain, and why for me?
  • Which system and brand — and can it be programmed in my country?
  • Rechargeable or not, and when would the battery need replacing?
  • What is likely to improve, and what probably will not?

Вопросы

How much does deep brain stimulation cost in India?

PACE Hospitals publishes about ₹5–18 lakh or more (about US$5,300–18,900). One-sided systems cost least; rechargeable and directional systems cost most.

Does DBS cure Parkinson’s disease?

No. DBS can ease tremor, stiffness and the swings between doses, and often lets people take fewer tablets, but the disease continues to progress.

How long do I need to stay in India for DBS?

We usually plan three to four weeks for assessment, surgery, recovery and first programming. Make sure a neurologist at home can program your device brand.
