Around month four, most athletes feel ready. The knee looks normal, the swelling is gone, and the team is winning without them. This is the most dangerous point of an ACL recovery — because the knee feels better long before it is strong enough. The research is clear: the calendar is not the test. Strength, control and patience are.
Why time and tests both matter
A new ACL graft goes through a slow remodelling process, and the muscles around the knee take months to recover after surgery. Returning early to cutting and pivoting sport puts load on both before they are ready.
Two studies published in the British Journal of Sports Medicine in 2016 changed practice. In the Delaware–Oslo cohort of 106 athletes in pivoting sports, the reinjury rate fell by 51% for each month return to sport was delayed, up to nine months after surgery. More symmetrical quadriceps strength before return also cut reinjury.
| Failed return-to-sport tests | 38.2% |
|---|---|
| Passed return-to-sport tests | 5.6% |
Grindem et al., BJSM 2016;50:804–808 (106 athletes). The difference did not reach statistical significance in this small group, but the direction matched other studies.
The second study followed 158 male professional athletes. Those who did not meet all six discharge criteria before returning to team training had a graft-rupture risk four times higher. A weaker hamstring-to-quadriceps ratio on the operated side also raised risk.
The message for athletes, coaches and clubs is the same: build the rehab around measurable targets, and do not let the calendar — or the fixture list — decide.
Week-by-week milestones
Every surgeon and physio uses their own protocol, and a meniscus repair or cartilage work done at the same time changes the limits. These are typical targets, not instructions. Your surgeon sets yours.
- Weeks 0–2control swelling, straighten the knee fully, wake up the quadriceps, walk with crutches
- Weeks 2–6walk normally without crutches, bend towards 120°, static bike
- Weeks 6–12strength work, single-leg balance, step-downs with good knee alignment
- Months 3–4straight-line running once strength and swelling criteria are met
- Months 4–6jumping and landing drills, then planned changes of direction
- Months 6–9sport-specific training, then non-contact team sessions
- 9 months +return to competition after passing strength, hop and confidence tests
Milestones are criteria-led: you move on when you meet the targets, not just because the weeks have passed.
The tests that clear you to play
A good return-to-play battery looks at strength, movement and readiness, not only at how the knee feels.
- Quadriceps and hamstring strength: measured on a machine where possible, comparing the operated leg with the other. Many programmes aim for at least 90% symmetry.
- Hop tests: single, triple and crossover hops for distance, and a timed hop, again compared side to side.
- Movement quality: landing and cutting with the knee tracking well, not collapsing inwards.
- Psychological readiness: questionnaires about fear of reinjury. Confidence is part of fitness.
- Sport-specific drills: controlled exposure to the real demands of your sport before full contact.
Overall, a 2014 meta-analysis found that 81% of people returned to some sport after ACL reconstruction, 65% to their pre-injury level and 55% to competitive sport. Those numbers are why structured rehabilitation deserves as much attention as the operation.

Doing part of your rehab in India
Athletes who have surgery in India usually stay about two weeks for the operation and the first phase, then continue at home. Others come back for a focused block — for example, months three to five, when running and jumping are introduced and supervision makes the biggest difference.
- Daily physiotherapy with a therapist used to athletes, plus strength-testing where available.
- Clear handover: written test results and the next phase's plan for your club physio.
- Somewhere pleasant to train: Chandigarh's flat lakeside paths and the cooler Himalayan foothills nearby — see rehab in the foothills.
- Long stays: our Hoppers stays start from US$999 a month with three meals.
For coaches and clubs
Pressure to return usually comes from outside the knee. Agree the criteria with the player, the surgeon and the physio before rehab starts, and write them down. That turns a difficult conversation in month five into a simple check: has the athlete passed the tests or not?
- Build training load gradually: sessions, minutes and intensity should rise step by step after clearance, not jump straight to a full match.
- Keep prevention going: neuromuscular warm-ups such as FIFA 11+ were linked to fewer knee injuries in football trials, and they cost only a few minutes a session.
- Watch the other knee: the uninjured side is also at risk, so strength and landing work should cover both legs.
- Plan the first match: a planned return, perhaps off the bench, is easier on body and mind than a sudden start.
The aim is not just to return, but to stay back.
Also check medicines with your team doctor if your athletes are drug-tested — read anti-doping and treatment abroad. For the surgery itself, see our ACL reconstruction guide and the sports medicine guide, or book a consultation.

When can I return to sport after ACL surgery?
Many sports surgeons now suggest at least nine months before pivoting sport, and only after passing strength, hop and readiness tests. One study found reinjury fell 51% for each month of delay up to nine months.
What tests are used before return to play after ACL reconstruction?
Quadriceps and hamstring strength compared with the other leg, hop tests, movement quality, psychological readiness and sport-specific drills.
When can I start running after ACL surgery?
Often around months three to four, once swelling is controlled and strength targets are met. Your surgeon and physio decide.
