Fast Bowler Injuries: Back Stress Fractures and Side Strains

Cricket fast bowlers' injuries explained: lumbar stress fractures, side strains and shoulder problems — diagnosis, rehab and treatment in India.

He is nineteen, quick, and the coaches love him. Then the back pain starts — one side only, worse when he arches backwards, eased by rest and back the moment he bowls. Every cricket physio has seen this story. Fast bowling is one of the most demanding movements in sport, and the young men and women who do it carry a particular set of injuries. Here is what they are, and how they are treated.

Why fast bowlers get hurt

A fast bowler runs in, leaps, lands on a braced front leg and whips the body round and down, all in a fraction of a second. The lower back bends, extends and twists under several times body weight, ball after ball, over after over.

A decade of Australian elite injury surveillance, published in 2016, showed how heavily that lands on pace bowlers. On average, 20.6% of fast bowlers were unavailable through injury at any given time, compared with 12.5% across all players. The single most prevalent injury was a lumbar stress fracture.

Young bowlers are the most vulnerable. Their spines are still maturing, their workloads can jump sharply between school, club and academy cricket, and their actions are still being shaped.

Lumbar stress fractures

A lumbar stress fracture in a fast bowler usually affects a small bridge of bone at the back of a lower vertebra, the pars interarticularis. It typically appears on the side opposite the bowling arm. Doctors call it spondylolysis.

  • Symptoms: one-sided low back pain that builds with bowling and backward bending, often in a teenager or young adult.
  • Diagnosis: MRI is often preferred in young athletes because it can show early bone stress without radiation; CT may be used to see whether a fracture has healed.
  • Treatment: time away from bowling, a graded rehabilitation programme for the trunk, hips and legs, then a carefully planned return to bowling workloads. Surgery is uncommon and kept for fractures that do not heal.

The hard part is patience. The pain often settles long before the bone has recovered, and bowling too soon is how a stress reaction becomes a fracture — or a fracture fails to heal. Lumbar stress fractures accounted for 15% of all playing time missed in the Australian study.

Side strains

A side strain is a tear of the muscles on the side of the trunk, most often the internal oblique where it attaches to the 11th rib. In fast bowlers it occurs on the side opposite the bowling arm, as the front arm is pulled down hard at delivery. It feels like a sudden, sharp pain low on the side of the chest.

MRI is the preferred scan. Reported return-to-play times range from one to 70 days, and recurrences within the same season are common. A sports physician will guide rehab by symptoms and function rather than by the scan alone, rebuilding trunk rotation and bowling volume step by step. Players under 24 were about twice as likely to suffer one as those aged 25 to 29.

Shoulders, knees and ankles

Fast bowlers also load the front knee and ankle at landing, and the bowling shoulder through thousands of deliveries. Fielding brings dives, slides and throws. Common problems include:

  • front-ankle impingement and ankle sprains from landing;
  • patellar tendon pain and meniscus injuries in the front knee;
  • shoulder pain and labral injuries from throwing and diving — see our shoulder surgery guide;
  • hamstring strains, the most common new injury in the same Australian study.

Treatment in cricket's home

Few countries see as many young cricketers as India, and its cricket medicine is growing fast. In September 2024 the BCCI opened its Centre of Excellence in Bengaluru, with three grounds, 86 pitches and a dedicated sports science and medicine facility. In the Himalayan foothills, Dharamshala's HPCA stadium sits at 1,457 metres — a reminder that India can also offer cool-weather places to rebuild bowling fitness.

These are cricket bodies, not BookMedTrip partners. What we arrange is a review of your scans by a sports specialist, treatment where it is needed, and a rehab plan your coach at home can follow.

Why do fast bowlers get back stress fractures?

Fast bowling bends, extends and twists the lower back under heavy load many times per session. Young bowlers with maturing spines and sharp rises in workload are most at risk.

How long does a side strain take to heal in cricket?

Reported return-to-play times range from one to 70 days. Rehab is guided by symptoms and function, and same-season recurrences are common.

Do lumbar stress fractures in bowlers need surgery?

Rarely. Most are treated with rest from bowling, structured rehabilitation and a graded return to bowling. Surgery is kept for fractures that do not heal.
