Slipped Disc and Sciatica: Physio First, Surgery If Needed

Slipped disc and sciatica explained: why physiotherapy usually comes first, what the evidence says about surgery, warning signs and options in India.

The pain started with a sneeze, or lifting a suitcase, or nothing at all. Now it runs from the lower back down one leg, like a hot wire. Many people with a slipped disc are told “surgery” within weeks. The evidence says: slow down, ask good questions, and give your body a fair chance first.

What a slipped disc really is

Between the bones of your spine sit soft, rubbery discs. With age and strain, the outer ring can crack and the jelly-like centre bulges out. Doctors call this a herniated or prolapsed disc. When it presses on a nerve root in the lower back, pain, tingling or numbness travels down the leg. That leg pain is sciatica.

Two facts bring most patients relief on their own. First, MRI scans of people with no pain at all often show bulging discs, so the scan is only part of the story. Second, the body often shrinks and reabsorbs the bulging material over time. The NHS says sciatica usually gets better in a few weeks to a few months.

Physiotherapy and time come first

For most people the first plan is not an operation. It is movement, guided well:

  • Stay active. Long bed rest slows recovery. Short walks, often, are better.
  • Physiotherapy. Exercises that calm the nerve and strengthen the trunk, plus manual therapy where it helps.
  • Pain relief. Your doctor chooses medicines for nerve pain carefully; ordinary painkillers help less than people expect.
  • Heat, posture, sleep. Small things that make the exercises possible.

If severe leg pain continues, an epidural steroid injection — placed near the irritated nerve under imaging — can calm it enough for rehabilitation to work.

Some international patients choose to do this part in India too. A two- to four-week block of daily, supervised physiotherapy in a calm city — with a spine specialist checking progress each week — can be easier to commit to than squeezing sessions around work at home. In Chandigarh, you can add gentle stretching guided by your physiotherapist and long, flat walks around Sukhna Lake. It is not a cure, and we will not promise one; it is simply a fair, structured chance for the nerve to settle.

A useful rule of thumb from spine surgeons: judge progress by what you can do, not by the scan. Walking further, sleeping through the night and sitting longer are the signs that matter. Keep a simple diary of these, and bring it to every appointment.

When surgery makes sense

A large 2023 review in The BMJ pooled 24 randomised trials. Disc surgery (discectomy) relieved leg pain faster than non-surgical care, but the advantage faded with time, and the evidence was rated low certainty. Surgery and injections gave similar leg-pain results.

So surgery is a reasonable choice when relief matters most and pain is not settling — and an urgent one in a few situations. Go to an emergency department at once if you notice:

  • numbness around the genitals or bottom;
  • new trouble passing urine, or loss of bladder or bowel control;
  • weakness or numbness getting worse in both legs.

These can be signs of nerve compression that needs hospital treatment the same day — do not plan travel; get care where you are.

Treatment options in India

Indian spine centres offer the full range, from physiotherapy to minimally invasive surgery:

  1. Specialist review: a spine surgeon and, ideally, a pain specialist look at your MRI and examination together.
  2. Structured rehabilitation: two to four weeks of supervised physiotherapy if surgery is not needed yet.
  3. Injections: epidural or nerve-root blocks done as day procedures.
  4. Microdiscectomy or endoscopic discectomy: removing the piece of disc pressing on the nerve, often with one or two nights in hospital.
  5. Fusion or disc replacement: for a smaller group with instability or repeated problems — a bigger decision that deserves a second opinion.

For a microdiscectomy, we plan about ten days to two weeks in India so you can be checked and start physiotherapy before flying. Prices are per person and exclude flights.

Questions to ask before you agree

  • Does my MRI match my symptoms, or could something else explain the pain?
  • What happens if I wait six more weeks with physiotherapy?
  • Would an injection be a sensible step first?
  • Which operation, how long in hospital, and when can I sit on a long flight?
  • What is the chance the disc slips again, and how do I lower it?

We will never push you towards surgery. A US$99 consultation includes two calls — a follow-up with the same specialist or a second opinion from another. Read our spine and neuro guide, why we don’t push surgery, and our second opinion guide, or send us your MRI.

Does a slipped disc heal without surgery?

Often, yes. The NHS says sciatica usually gets better in a few weeks to a few months, and most people recover with activity, physiotherapy and pain relief.

When is slipped disc surgery needed?

When leg pain stays severe despite good non-surgical care, or urgently if there is numbness around the genitals, bladder or bowel problems, or worsening weakness in both legs.

How much does a microdiscectomy cost in India?

Published ranges are about ₹1.2–4 lakh (about US$1,260–4,200). Your written estimate depends on the technique, hospital days and room.
