Angioplasty vs Bypass: Choosing a Stent or Surgery

Stent or bypass? How cardiologists choose between angioplasty and heart surgery, what India’s NPPA stent price caps mean, and recovery for each.

The angiogram is done. The cardiologist turns the screen and points: three narrowed arteries. Then comes the question every family dreads — stent or bypass? Both are good treatments. Neither is right for everyone. Here is how doctors choose, what India’s stent price caps mean for you, and what recovery looks like.

Two ways to restore blood flow

Angioplasty with a stent (PCI) is done through a small tube in the wrist or groin. A balloon opens the narrowing and a tiny mesh tube — the stent — holds it open. Most modern stents are coated with medicine to stop the artery narrowing again. You are awake, and you are usually home within a day.

Bypass surgery (CABG) is open-heart surgery. The surgeon takes a healthy artery from the chest wall or arm, or a vein from the leg, and uses it to route blood around the blockages. It is a bigger operation with a longer recovery, but it can treat several blockages at once and often lasts very well.

Families often ask which is “safer”. In the short term, angioplasty is the smaller procedure, with a faster return home. Over the long term, the answer depends on how extensive the disease is — which is why the angiogram, not the brochure, should decide. Ask the cardiologist to show you the images and explain each narrowing in plain words; a good one will be glad to.

For some people, the best answer is neither — good medicines and lifestyle change. A careful cardiologist will tell you if that is your situation.

How doctors choose

The decision depends on your anatomy and your health, not on which team you meet first. In general:

  • Stents are often preferred for one or two narrowed arteries, simple blockages, or an emergency heart attack, where opening the artery quickly saves heart muscle.
  • Bypass is often preferred for disease in all three main arteries, a narrowed left main artery with complex disease, a weak heart pump, or diabetes with multivessel disease.
  • Your other conditions matter: kidney function, lung disease, age, frailty and your own preferences all weigh in.

The landmark FREEDOM trial followed 1,900 people with diabetes and multivessel disease for five years.

The best hospitals discuss such cases in a “heart team” meeting, where an interventional cardiologist and a cardiac surgeon look at the same angiogram together. Bring every report you have: the angiogram CD (not only the written report), an echocardiogram showing how well your heart pumps, kidney function tests, and your medicines list. If you have had a heart attack, bring those hospital notes too. Ask whether yours has been reviewed that way — and remember that a US$99 consultation gives you two calls, so a second specialist can look at the same CD.

India’s stent price caps

In February 2017 India’s National Pharmaceutical Pricing Authority capped stent prices. Before that, stents sold for between ₹25,000 and ₹2 lakh each. The cap is revised each year in line with inflation.

Before-cap range: Business Standard, 15 February 2017. Ceiling prices exclude GST: NPPA, via Medical Dialogues, 7 April 2026. A bare-metal stent is capped at ₹10,762.

The cap applies to the stent itself, not the whole procedure. Your hospital bill also covers the catheter lab, balloons, wires, the doctor’s fees and your stay. Check that the stent appears on your estimate and bill at or below the ceiling price, with its brand and batch number.

Recovery and flying home

After angioplasty: the NHS says most people with angina go home the same day or the next; after a heart attack the stay is longer. Avoid heavy lifting, strenuous activity and driving for at least a week. We plan about a week in India so your cardiologist can check the access site and your medicines before you fly.

After bypass: usually several days in hospital, including intensive care, then a gentle recovery. We plan three to four weeks in India in total, with cardiac rehabilitation walks before your surgeon clears you to fly.

  • Take your blood thinners exactly as prescribed. Stopping them early after a stent can be dangerous.
  • Carry your stent card or discharge summary whenever you travel.
  • Join a cardiac rehabilitation programme at home — it lowers the risk of future problems.
  • Seek urgent help for chest pain, breathlessness or fainting.

پرسش‌ها

Is a stent better than bypass surgery?

It depends on your arteries and health. Stents suit simpler disease and emergencies; bypass is often preferred for three-vessel or left main disease, a weak heart or diabetes with multivessel disease.

How much does a stent cost in India?

From 1 April 2026, India caps a drug-eluting stent at ₹39,186 and a bare-metal stent at ₹10,762, before GST. The full procedure costs more.

How long after angioplasty can I fly?

Most people go home the same or next day. We plan about a week in India so your cardiologist can check you before the flight.
