ACL Reconstruction in India: Surgery and Rehab Timeline

Sports knee injuries and ACL reconstruction in India: when surgery is needed, a two-week treatment plan, costs, and the honest 9-month rehab timeline.

A sudden twist on the football pitch, a loud pop, and a knee that swells like a balloon within hours. For many young athletes — and plenty of weekend players — that is how an anterior cruciate ligament (ACL) tear begins. Here is what happens next: diagnosis, whether you need surgery, what ACL reconstruction involves, and the long, honest road back to sport.

Common sports knee injuries

The knee depends on four main ligaments and two crescent-shaped cartilage cushions called menisci. Sports that involve cutting, jumping and landing — football, basketball, netball, rugby, kabaddi, skiing — put them under great strain.

  • ACL tear: often a non-contact twist; the knee may give way later on turns.
  • Meniscus tear: pain along the joint line, catching or locking; often occurs with an ACL tear.
  • MCL sprain: a blow to the outside of the knee; most heal with a brace and rehabilitation.
  • Cartilage injury: damage to the smooth joint surface, sometimes found only at arthroscopy.

Diagnosis combines a specialist’s examination (tests such as the Lachman test) with an MRI scan. Send us your MRI images, not just the report — the surgeon will want to see them.

Do you need surgery?

Not always. Some people with an ACL tear — especially those who do not play pivoting sports — do well with a structured rehabilitation programme alone. Surgery is usually advised when the knee keeps giving way, when you want to return to cutting and pivoting sport, or when there is a repairable meniscus tear alongside.

Timing matters too. Most surgeons prefer to operate once the swelling has settled and you can bend and straighten the knee well — often a few weeks after injury. Prehabilitation before surgery (rebuilding movement and thigh strength) tends to make recovery easier.

Age is not a barrier on its own. Surgeons look at your activity level, the state of the rest of the knee and your goals. For children and teenagers whose growth plates are still open, special techniques protect growth — ask a surgeon experienced in young athletes. For people in their forties and fifties who want to keep playing tennis, trekking or skiing, reconstruction can still be a good option.

What ACL reconstruction involves

ACL reconstruction is keyhole (arthroscopic) surgery. The torn ligament is replaced with a graft — usually taken from your own hamstring, patellar tendon or quadriceps tendon — fixed into tunnels drilled in the thigh and shin bones. Any meniscus tear is repaired or trimmed in the same operation.

  1. Day 1: examination, MRI review, blood tests and a meeting with the surgeon and physiotherapist.
  2. Day 2: surgery, usually under spinal or general anaesthesia, lasting one to two hours.
  3. Days 3–4: walking with crutches and a brace; first physiotherapy sessions focus on straightening the knee fully.
  4. Days 5–14: daily physiotherapy near the hospital, wound check, and a written rehabilitation plan for home.

We plan about two weeks in India. Your surgeon decides when you can fly and what blood-clot prevention you need on the plane.

The rehabilitation timeline

The operation is the easy part. Rehabilitation takes most of a year, and it is what decides how well you return.

  • Weeks 0–6: full straightening, bending to 90 degrees and beyond, walking without crutches, controlling swelling.
  • Months 2–4: strength work — squats, step-ups, cycling — and balance training.
  • Months 4–6: running in straight lines, then jumping and landing drills, if your physiotherapist agrees.
  • Months 6–9: sport-specific training, cutting and pivoting, building confidence.
  • From about 9 months: return to competitive sport — only after passing strength and hop tests that compare the two legs.

Those numbers are honest, not discouraging. Many people do return to the sport they love; others change sports or levels. In a Norwegian–American study, the re-injury rate fell by 51% for each month return to sport was delayed, up to nine months after surgery, and more equal thigh strength between the legs also lowered the risk. Patience protects the new ligament.

The practical lesson: plan the whole year, not just the operation. Line up a sports physiotherapist at home before you fly, so your first session is booked for the week you land. Share your Indian surgeon’s operation note and rehabilitation plan with them, and send progress videos back to India at the key milestones. Our care team helps keep both sides talking.

Questions to ask your surgeon

  • Do I need surgery, or could rehabilitation alone work for my sport?
  • Which graft do you recommend for me, and why?
  • Is there a meniscus tear, and will you repair or trim it?
  • How many ACL reconstructions do you do each year?
  • What tests must I pass before returning to sport, and who will do them at home?

Prices are per person and exclude flights; your estimate should name the fixation devices and the physiotherapy included. Read our joint and orthopaedic guide, our recovery roadmap and costs side by side, or send us your MRI — a US$99 consultation includes two calls with a sports-knee surgeon.

How much does ACL reconstruction cost in India?

Published ranges are about ₹1.2–4 lakh (about US$1,260–4,200), depending on the graft, fixation devices, hospital and any meniscus repair.

How long does it take to return to sport after ACL surgery?

Usually at least nine months, and only after passing strength and hop tests. Re-injury risk falls the longer you wait, up to about nine months.

How long should I stay in India after ACL reconstruction?

About two weeks: surgery, early physiotherapy and a wound check, then a rehabilitation plan to continue at home.
